Literature Order Form, these prices are not correct, and will be needed to be updated to meet your area's literature prices, this is also only the general literature that our area purchases.
I would suggest keeping the namefields the same as the calculator plugin for this is very picky and may throw error. When changing the prices I'd suggest changing the price from the table section then searching the name for what's being added to the bottom calculator field.

Example: Change Basic Text Price: Find the Basic Text below (It would look like this)

Table Tutorial

Basic Text $12.25 [number BT min:0 placeholder "0"]
Calculator Tutorial.
[calculator total Prefix:$ Precision:2 "(BT*12.25)...."]
Mail Tutorial
If you make a change to these values also be sure to change the price on the Mail tab.

The E-mail sent does not have the ability to be HTML like the form field so it's just basic using the shortcode.
